As a registered volunteer for Apostolos Missions International’s local chapter at

and for all volunteer related events and activities, you do hereby discharge Apostolos Missions International, the event site, their management, officers, board members, employees, members, volunteers, representatives and their successors, and all cooperating organizations, from all claims of damages, demands, actions and causes whatsoever in any manner arising from or growing out of your participation in an Apostolos Missions International activity or event.

As a volunteer, you agree to indemnify and hold Apostolos Missions International harmless for all fines, penalties, fees and expenses incurred as a result of or related to any breach of contractual obligation to the participant.

By completing and submitting this form to Apostolos Missions International you understand and agree to the following:

• I will conduct all volunteer activities with high standards and professionalism, and will do nothing to cause detriment to the reputation or goodwill of Apostolos Missions International (AM).

• I agree and support the AM Mission Statement and Statement of Faith.

• I declare the information provided by me in this Application is true, correct and complete, and intend to be legally bound by its terms.

Apostolos Missions International will evaluate what you have written and may ask you to provide additional information. In addition, Apostolos Missions International may determine that it needs to conduct an investigation and may require your assistance with this. This investigation may include a background screening including, but not limited to, the following: National Criminal Database, Social Security Search, National Sex Offenders Registry, and Motor Vehicle Records. While checking “yes” to any of the questions above will not result in automatic disqualification or termination from Apostolos Missions International, Apostolos Missions International may determine that an affirmative response would make an individual unfit for service or association with Apostolos Missions International’s ministry. In all cases, Apostolos Missions International reserves the right to determine, at its discretion, what action should be taken.

If any of your responses or information provided on this form are untruthful or inaccurate, Apostolos Missions International may determine that you are no longer qualified to be associated with the ministry in any capacity.

By signing below, you agree that, if you become an employee or volunteer of Apostolos Missions International, and become involved in criminal proceedings or other circumstances that would cause your answers to any of the questions above to change, you will immediately notify, and provide a complete description of the circumstances, to the Director of Human Resources of Apostolos Missions International.

To serve with Apostolos Missions International it is required that you sign below, indicating your agreement with the Consent and Release. You must also sign the Statement of Agreement, which includes Apostolos Missions International’s Purpose and Doctrinal Basis. Mail the signed hard copies with your application, or sign using the electronic signature format outlined at the beginning of the application.
